Pricing by age 

• 0-18 years old: $50/month

• 19-49 years old: $100/month

• 50-89 years old: $150/month

• 90+ years old: $90/month

• Family max: $300/month

• Enrollment Fee: $100/patient or $300/family (waived if enrolled before 05/01/2024)

Pricing for options not included in regular membership:

* Fourth-trimester and newborn (available soon): $200/month for 1st 3 months then $50/month for newborn and $100/month for mother after the 3rd month; no enrollment fee

* Nursing home/Rehab/Assisted Living (available soon): $150/month

* Substance Use Disorder treatment: $150/month plus medication costs (as needed).

* Urgent Care visit (non-member, 2 visit limit): $150/visit

* Medical-grade Piercing: starting at $100, depending on the service requested.
